Coming Home From the Hospital: How to Start at Oak Knoll

If the hospital says your loved one needs skilled nursing next, you do not have to figure it out alone. Discharge days move fast, and the paperwork can wait a few minutes while you get your bearings.

Oak Knoll Skilled Nursing & Rehabilitation Center is here for those first calls and visits. When you are ready for the checklist and paperwork, use our Admissions page.

The hospital team recommends skilled nursing or short-term rehab. You (or the case manager) call Oak Knoll admissions at 314-521-7419 to talk through needs and timing. Tour if you can. Weekdays are open 9 to 5; weekends are by appointment. When you are ready for documents and insurance detail, go to Admissions.

We do not promise same-day admission or a set clock for when a bed will open. Admissions will tell you what is possible for your loved one’s situation.
